New Patient Information
Dr. Young asks that new patients complete 3 pages of simple forms prior to being seen for the first time. If time has been allowed in scheduling, we will provide for you, and you may fill out these forms in the office immediately prior to your appointment. If you have been asked to complete the forms in advance, please click on the following link, print out the 3 pages, and bring the completed forms to the office for your first visit.
